- 兒童的香港出生證明書（正本及影印本）（若在出生證明書上最後一欄顯示兒童的香港本土人士身份「未經確定」，家長必須帶備兒童的旅行證件或在港居留許可證的正本及影印本，如非本港出生者，必須遞交單程証及相關文件副本）或外地出生證明書及護照（正本及影印本）；
